Social Security Number (SSN)

The PSP-13 form requires the inclusion of the test-taker's SSN. We would not ask if it was not required by the state. When you have concluded your test and your form has been generated, our system will purge your SSN. If you do not pass the test, and choose to not re-take it, you may request that we delete your SSN from our system.
